How to Onboard
Complete the Carrier Onboarding Form below. It takes less than 5 minutes. You will need your MC#, USDOT# and insurance information ready.
Upload your Certificate of Insurance (COI), W-9 and MC Operating Authority certificate directly through the secure portal.
Review and digitally sign the Qivits Broker-Carrier Agreement. Once approved, you are in our active carrier network and eligible for load assignments.
